Although, if there is no obstruction present, an infection can sometimes be cured in a day or two, doctors often ask a patient to continue with their treatment for 10 days or 2 weeks so as to ensure a cure. If there are indications of kidney infection, or where there is known diabetes or where treatment has been delayed for some reason then long-term treatment is required. Sexually transmitted conditions such as Chlamydia or Mycoplasma will require longer term treatment, usually with tetracycline, and trimethoprim/sulfamethoxazole. Because symptoms may disappear some time before the infection is totally eradicated it must be stressed to the patient that they take the total amount prescribed and there should be a follow up urinalysis to make sure it is over, but this is rarely carried out, which may go a long way to explain why such infections can recur with frequency.
If the infection reaches the kidneys treatment may be needed over an extended period of several weeks. In some cases kidney infections that aren't properly treated may lead to permanent kidney damage or even kidney failure.
There maybe also symptomatic treatments -- e.g. giving extra fluids either by mouth or intravenously. Advising smokers to give up may help. Smoking is a common cause of bladder cancer and does cause irritation.
Where there are frequent attacks, common in women, they may be advised to take a single dose of antibiotic after sexual intercourse, or be on a low dose of a drug such as nitrofuradantin for as long as six months. This is taken once daily at night so that drug stays in the bladder for a longer period than if it were taken during the day. Dipsticks that detect infection immediately can now be obtained. Using these a patient can detect the infection for themselves and then use a short-term antibiotic to suppress it. They will revel 90% of such infections and can be a useful tool for women who have many infections.
